Available program: Mobile Crisis Residential

Overview: The goals of Community Solutions’ Crisis Residential Program are to successfully reintegrate the client back in the community by focusing on Recovery Based and Trauma informed services and supports that build safety, interpersonal and independent living skills as well as developing community support. Serves adults ages 18-59.

Location: Program/Client base throughout Santa Clara County and may include surrounding areas.

Special Requirements : Must be able to obtain and maintain program specific certifications and receive a criminal records clearance as required by specific programs, funding requirements and licensing regulations. Must be able to obtain and remain qualified for County 5150 Hold authorization. Requires an automobile to perform routine job duties and requires the willingness and ability to transport clients in your own vehicle.

License : Must be registered and in good standing with California Board of Behavioral Science (BBS) as a licensed Marriage Family Therapist Intern (MFTI), an Associate Clinical Social Worker (ASW) or eligible to register with the Board of Psychology as an Associate Psychologist (APSY) as a waivered therapist. Must apply for registration and waiver at time of hire, obtain waiver within four months of hire date and maintain registration and waiver. Possess a valid California driver’s license, current car insurance and a good driving record as documented by a DMV report and consistent with the Agency’s liability insurance requirements.

Knowledge Of:
Clinical knowledge and expertise within scope of practice in group, family and individual therapy.

Treatment, rehabilitation and assessment techniques for victims/survivors of violence and their families, people with mental illness or substance abuse issues.

Working knowledge of wellness and recovery concepts, Medi-Cal regulations and program documentation.

Knowledge of multiple partner systems, case management group and individual counseling, and linkage-referral, and program development as it relates to coaching teammates and supporting the program’s success.

Ability To:
Maintain assigned case load, achieve productivity expectations and work within a program structure.

Maintain good strength based problem solving and conflict resolution skills. Consistently applies good judgment.

Provide strength-based, client driven and outcome driven care, based on use of assessment and outcome tools.

Accurately assess clients’ needs and strengths.

Engage clients in the treatment process.

Effectively engages and interacts with clients and families demonstrating strong interpersonal skills. Handles all client interactions in a courteous and professional manner using effective reflective listening skills and seeks to understand others divergent views.

Essential Duties:
Utilizes clinical best practices to conduct screening, intake and assessments to determine the client’s needs develop treatment goals. Promptly develops and/or implements individual and family treatment plans using Transformational Care Planning and is responsible for coordination of care to provide for the client’s stated needs and goals. Community based work to include home visits and school based services.

Performs intensive counseling services with clients and their families through individual, family and group therapy. Employs evidenced-based practices and the wellness & recovery model as appropriate to assist the client and identify issues of dual diagnosis, activities of daily living, employment, mental illness, physical illness, relationships, and housing which are impacting their mental health and successful recovery.

Provides crisis intervention, counseling, advocacy, case management and referral services for assigned individuals and families.

Teaches life skills, assists individuals and families with educational and support activities, accesses appropriate community services and rehabilitation counseling.

Flexible work schedule may be required including evenings and/or weekends, based on client/program needs.
