SpEd Transition Teacher

Post by Jobing – Job Description The Bridges Transition Program consults with teams and serves students with disabilities from 8th grade through age 21.Must possess a Special Education Teaching License or a Special Service Provider License from Colorado Dept of Education.Minimum 5 years teaching or special service provider experience in Special Education.Career Technical Education certificate from Colorado Dept of Education preferred.Participate as a member of multidisciplinary teams in the evaluation and goal setting for students, with a focus on life skills, career, post-secondary education, and community independence.Coordinate Transition Team in providing career development and job placement for High School/Transition students.Coordinate post-secondary educational opportunities available to Transition students.Coordinate Transition Team in providing appropriate community/social/rec-leisure activities for Transition students.Coordinate the education of and linkages to adult agencies available in our community including participation in the local interagency network and regional coordinator collaboration.Train, oversee, and evaluate Transition paraprofessionals/coaches who assist in providing programming to Transition students.Coordinate all Transition staff who provide services to Transition students to ensure that IEP’s are being implemented.Provide case management support to Transition students and be ultimately responsible that IEP goals and objectives are met. Must possess an understanding and knowledge of Response to Intervention as it relates to Transition services.Willing to take and renew/maintain CPR, First Aid, and Nonviolent Crisis Intervention training.Able to lift 50 pounds each in a two person lift.Must be able to transport self to community agency events.Working knowledge of technology resources and software.Coordinate implementation of web-based job skills curriculum.Build relationships and communicate effectively with teachers, students and their families.Act independently with minimal supervision.Deep knowledge of compliance indicators for Transition age students (Indicator 13 and 14) and participation on District 20 Transition Team.Serve as Transition Liaison with high school teams. Job Requirements Teacher: Sped Transition: Valid Colorado Teaching LicenseDemonstrated success in modifying curriculumAbility to accurately assess need and provide a range of appropriate services to studentsExcellent interpersonal skills; skillful communicator with students, staff and parentsKnowledge of and experience with pre-referral interventions and behavior contracts, as well as IEP, ILP, 504 guidelines and proceduresExcellent organizational and academic skillsEffective student management skillsStudent centered; Demonstrated success in working with studentsExcellent collaborative/teaming skillsEnergetic, creative, innovative, flexibleAbility to use data/technology to improve instruction and learning, and to interpret test dataWorking knowledge of and success in applying state standards in subject areaHighly qualified per Colorado Department of Education and per district requirements, if applicable
